Somatic Therapy & Nervous System Regulation
Somatic Therapy for When Your Body Won’t Believe You’re Safe
The Body Knows
You can know, logically, that you’re safe — and your body still won’t cooperate. Your shoulders stay up around your ears. Your stomach stays in knots. You startle easily, or you go strangely flat and numb, and no amount of positive self-talk seems to reach whatever’s actually driving it. That’s because talk alone speaks to your thinking brain, while a dysregulated nervous system operates well below conscious thought. At Made Well Center for Wholeness, our somatic-informed therapists in Wilmington, NC (and online) work directly with your body’s stress response — not just your thoughts about it — to help your nervous system finally learn what your mind already knows: that it’s safe to settle.
You Might Be Here Because…
- You feel chronically tense — jaw, shoulders, stomach — even when nothing is actively wrong.
- You startle easily or feel constantly “on,” like your body is braced for impact.
- You go numb, foggy, or disconnected in moments of stress instead of feeling present.
- Traditional talk therapy has helped you understand your patterns intellectually, but your body hasn’t caught up.
- You have physical symptoms — tension, GI issues, headaches, fatigue — that doctors can’t fully explain.
- You struggle to feel calm even in objectively safe, relaxed situations.
- Breathing exercises and grounding tips help for a minute and then wear off fast.
- You feel disconnected from your own body, like you’re living entirely in your head.
- Certain sensations, touch, or physical closeness trigger a stress response you can’t logic your way out of.
- You’re ready for something that works with your body instead of around it.
What Nervous System Dysregulation Actually Feels Like
Your nervous system has a built-in stress-response system that’s meant to activate during real danger and then settle back down once the danger passes. For many people — especially trauma survivors, chronically stressed adults, and highly anxious individuals — that system gets stuck in an activated or shut-down state, even when there’s no current threat. This is often explained through polyvagal theory: your nervous system cycling between a mobilized “fight or flight” state, a shut-down “freeze” or collapse state, and a calm, connected “safe and social” state that can start to feel hard to reach. Somatic approaches use techniques like titration (working with small, manageable amounts of activation so the body doesn’t get overwhelmed) and pendulation (moving attention between tension and a felt sense of safety) to help your nervous system practice returning to that regulated state, rather than just thinking your way there.
How Made Well Works With You Differently
We integrate somatic, body-aware techniques directly into treatment rather than offering talk therapy alone, because for many clients — particularly those with trauma, chronic anxiety, or stress that “logic” hasn’t touched — the body needs to be part of the work, not an afterthought. Depending on your needs, this can include Somatic Experiencing-informed techniques, breathwork and grounding practiced in-session (not just assigned as homework), body awareness and titration work, and coordination with EMDR or Sensorimotor Psychotherapy when deeper trauma processing is appropriate. We move at a pace your nervous system can actually tolerate, building capacity and safety before asking your body to do anything more than it’s ready for. For clients who find it meaningful, we can also integrate contemplative or faith-based grounding practices, like breath prayer, into nervous system work.
What Therapy Here Looks Like:
1. Free consultation, or Email Scheduling — talk through what your body does under stress and what approaches might help
2. Whole-person Assessment— mapping your specific patterns of activation, shutdown, and what helps you feel safe.
3. A personalized treatment plan — combining somatic techniques with talk therapy, paced according to your nervous system’s actual capacity
4. Ongoing Support that adapts — you build regulation skills with support, not alone and unsure if you’re “doing it right.”
Who We Work With:
- Trauma survivors whose bodies stay activated even after the danger has passed
- Chronically anxious adults for whom talk therapy alone hasn’t been enough
- Clients with stress-related physical symptoms without a clear medical cause
- People who feel disconnected from or uncomfortable in their own bodies
- Clients already in trauma therapy (EMDR, Sensorimotor) wanting integrated body-based support
- Highly analytical or “in-my-head” clients ready to work with the body directly
FREQUENTLY ASKED QUESTIONS:
Scroll through our frequently asked questions below to learn more about how we might be the best fit to walk with you on this journey to healing and wholeness:
What is somatic therapy, exactly?
Somatic therapy is an umbrella term for body-based therapeutic approaches that work directly with physical sensation, nervous system activation, and the body’s stored stress responses, rather than relying only on verbal, cognitive processing. It’s often used alongside or integrated into trauma-focused treatment.
Why doesn't talk therapy alone fix my anxiety symptoms?
Talk therapy primarily engages your thinking brain, while chronic anxiety and stress responses are largely driven by your nervous system operating below conscious thought — which is why you can fully understand your anxiety intellectually and still feel it in your body. Somatic approaches work directly with that nervous system response.
What is polyvagal theory, and do I need to understand it to benefit from somatic therapy?
Polyvagal theory is a framework describing how your nervous system shifts between mobilized (“fight or flight”), shut-down (“freeze”), and calm, connected states — it’s a useful lens for understanding somatic work, but you don’t need to study the theory itself to benefit from the practical techniques.
Is it true that trauma gets "stored in the body"?
Many clinicians and researchers describe trauma as leaving lasting imprints on the nervous system’s stressresponse patterns, which can show up as chronic tension, physical symptoms, or activation long after the triggering event — this is the basis for why body-based, not just cognitive, treatment is often necessary for full recovery.
What actually happens in a somatic therapy session?
Sessions typically involve tracking physical sensations, practicing grounding and regulation techniques in real time, and working with small, manageable amounts of activation (titration) so your nervous system can build tolerance without becoming overwhelmed — it’s guided, gradual, and paced to your capacity.
Is somatic therapy the same as Somatic Experiencing or Sensorimotor Psychotherapy?
Somatic therapy is the broader umbrella; Somatic Experiencing and Sensorimotor Psychotherapy are specific, trademarked clinical approaches within that umbrella, each with their own training and technique. We can help clarify which specific approach fits your situation.
Why do I go numb or shut down instead of feeling anxious under stress?
This is a freeze or shutdown response — a nervous system state associated with feeling overwhelmed or unable to fight or flee — and it’s just as much a stress response as anxiety or panic, even though it looks and feels very different. Somatic work addresses both activation patterns.
Can somatic therapy help with physical symptoms my doctor can't explain?
Often, yes — chronic stress and nervous system dysregulation can produce real physical symptoms (tension, GI issues, fatigue, pain) without a clear structural medical cause, and addressing the underlying nervous system state can meaningfully reduce these symptoms alongside appropriate medical care.
Do I have to talk about traumatic memories in detail for somatic therapy to work?
Not necessarily — much of somatic work focuses on present-moment body sensations and nervous system patterns rather than requiring detailed narrative recounting of traumatic events, which is part of why it can feel more tolerable for some clients than traditional talk-based trauma processing.
Is somatic therapy safe if I tend to get overwhelmed easily?
Yes — a core principle of somatic work is titration, meaning we work with small, manageable amounts of activation at a time specifically so your system doesn’t become overwhelmed, and we build safety and resourcing before doing any deeper activation work.
Can somatic therapy be combined with EMDR or other trauma treatments?
Yes — somatic techniques are frequently integrated alongside EMDR, Sensorimotor Psychotherapy, or standard talk therapy, and combining approaches is common and often clinically beneficial rather than an either/or choice.
